Notfallblatt Bern

  1. Ausfüllen. Was nicht zutrifft, leer lassen.
  2. Drucken und an den Kühlschrank hängen. Rettungsdienste schauen dort zuerst nach. Eine Kopie ins Portemonnaie.
  3. Bei jeder Änderung der Medikamente neu drucken. Das Datum steht unten rechts.
Alles bleibt auf diesem Gerät. Deine Angaben gehen an keinen Server. Wenn du „Auf diesem Gerät merken“ einschaltest, speichert dein Browser den Entwurf lokal, sonst ist er nach dem Schliessen weg.

Emergency sheet for the fridge

Fill in the emergency sheet and stick it on the fridge: medicines, allergies, contacts for the ambulance crew in Bern. Your entries stay on your device.

The emergency sheet is a sheet for the fridge that gives the ambulance crew the most important answers: Which medicines? Which allergies? Who should be called? It helps even when the person can no longer answer themselves. It can be filled in by the person concerned or by a family member. The template is free to use, in Bern as in any other municipality.

The form asks for the person, the address and access to the flat (floor, door code, key), the health insurer, blood group and weight. Under “Important for paramedics” you tick what applies, for example blood thinners, pacemaker / ICD, diabetes, dementia or allergy to medicines. Then come a medicines table with dose and time of intake, diagnoses and operations, contacts, family doctor, pharmacy and Spitex home care, as well as advance directive, power of attorney and organ donor card, with where the documents are kept.

The preview shows the sheet as it will be printed, and “See an example” shows a filled-in sample. With “Print / save as PDF” you print it on A4, on a phone via Share and Print. Put it on the fridge, because ambulance crews look there first, and keep a copy in your wallet. Print it again whenever your medicines change; the date is at the top right.

Everything stays on your device; your entries are not sent to any server. Only if you switch on “Remember on this device” does your browser keep the draft locally. The sheet replaces neither the emergency number 144 nor a conversation with your doctor.
